Hepco & Becker skid plate - HELP

I bought a used Hepco Becker skid plate some time back, and am now getting around it putting it on. Unfortunately all the mounting hardware got mixed in with other nuts & bolts.

Can anyone sent me a photo of the hardwere (& measurements) so i can sort it out.

A copy of instructions would help also.
