Good afternoon from the sunny northeast,

I need to add a back cushion to the OEM top case to make the ride a bit more comfortable for my wife. I have been looking for gel or foam but haven't found anything that looks good. Anyone else found a solution? Thanks.

I assume their jackets already have padded CE armor?

I ordered a SW Motech - Backrest for their 38L Trax Alu-Box. It should be arriving in the next few days and I'll do some pics of the install.

I looked high and low for a backrest that I thought would fit onto the factory Yamaha topcase. A Vicious Cycle suggested I try this one:

It should be straight forward. I also have the SW Motech Trax Handles coming for the Yamaha factory sidecases:

I am an Army Guy, and was planning on having a Mat Tech (Materiels Technician) whip up a pad with velcro on it. I will post pics once I have it done.

3" dense foam and Matt black vinyl. Used a piece of hardboard
to staple into. Strip of Velcro on back and top box. Wife loves
it so now I will have the Vinyl fitted and stitched rather than
staples.

Just make em lean on the bare box! As someone said earlier they are wearing a padded jacket anyway so why do they need the extra comfort. If you make them too comfortable they go to sleep on the back. Thinking about it that at least stops her talking for a while so perhaps a backrest isn't such a bad thing :D

Not EOM, but not that far off. Pad was leftover from an FJR backrest. Spkldoe likes this set up pretty well:
