Installed a Givi trekker top case with back rest pad for the mrs and the box tilts forward putting her forward a bit. Has any one else seen this? To resolve this, I'm thinking about shimming the front of the mounting bracket up which would angle the top of the box back a bit. Or is there a mount that extends back a couple inches further?

I had the same problem with givi 52l box and have now fitted it to an altrider rack which makes it 2 inches further back

If she wears a padded jacket , you don’t need the pad on the case. It creates more room without it, I never installed our pad on the Trekker as it took too much room , it’s still sitting on the parts shelf.

Ran into the same problem with mine. I ended up with the Altrider rack but tilted it back and lowered it. I also removed the backrest from the top box. It had a terrible shape to it and my wife found it better with it removed. I don't have a close up of it but this is a pretty large picture.IMG_3411.jpegIMG_3411.jpeg

On my first Tenere I mounted the box on a thick HDPE plastic kitchen cutting board, then mounted the board to the rack 3" (? iirc) aft.

I also flattened the rear seat with a couple of spacers cut from another cutting board to hold up the front about a half inch. She said it made a world of difference for our longer rides.

The Altrider rack usually does the job, as others have suggested. I know some DIY modify the existing hardware, but in some jurisdictions this has insurance implications. That's down to the OP to check.

Some may still find space limited, dependent on physical size & gear worn. If your pillion has a back protector, then you may find the Givi backrest merely serves to close down the space available; indeed it's probably superfluous.
